The Comprehensive Spine Care Program Specialist ensures that patients are expediously seen by the appropriate provider in the program for care, Patients will be assessed to coordinate their care plan based on the provider’s evaluation. This care plan coordination may include but not limited to establishing care with another provider, rehabilitation services, behavioral health specialists, and imaging services.  When a surgical intervention is necessary, this position must ensure that patients are prepared and cleared for surgery, with the necessary diagnostic results and other relevant paperwork at least 7-14 days prior to surgery.  This position works collaboratively with emergency departments, home health coordinator, physicians, case management, nursing and rehabilitation services to achieve desired patient outcomes through cost-effective use of healthcare resources.  The Comprehensive Spine Care Specialist  assists in tracking, monitoring and improving outcomes such as post-operative complications, satisfaction and cost-effectiveness. This individual will also be involved in marketing intiatives for the program.

Collaborates with the case manager and other key members of the care team to determine key barriers or obstacles for recovery and facilitates the establishment of a  plan.
Collaborates with specialist, surgeon, patient and significant others in the establishment of a plan
Ensures that insurance benefit information is obtained and utilized in advance in formulating plan
Educates patient on care plan pathway and expectations. 
Documents key data elements per protocol
Communicates with patients and providers to set expectations and educate regarding care plan
Problem solve variances with specialist, surgeon, case manager, nursing and rehab staff
Ensures that the plan of care and information are linked between continuum provider

Works with physician liaison for musculoskeletal service line and neuroscience service line to market and promote program to potential referral sources as well as target patient populations

OTHER DUTIES AND RESPONSIBILITIES

The Comprehensive Spine Care Specialist has frequent and regular contact with patients, significant others/caregivers, physicians, nursing personnel, case managers, rehab staff and other ancillary providers.  The Specialist must act as a patient advocate to ensure that the care experience is patient and family centered.  This position will have a close working relationship with the Comprehensive Spine Program physician leaders as well.  The Comprehensive Spine Care Specialist will also have frequent community contacts

REQUIRED EDUCATION, EXPERIENCE AND SKILLS

Bachelors Degree required.
A current RN or Physical Therapist license is required.  
Special Certification in Spine, Pain, or Neuro is preferred.
Patient care experience in pain management or spine is preferred. Previous patient group teaching and education experience is also preferred. 
Experience working with a large and diverse group of healthcare providers is preferred. 
Knowledge of the spine care and pain management is preferred.   
Knowledge of the pathophysiology of joint disease and trauma and associated patient care is also required.
Knowledge of physical and psychosocial patient assessments required.  
Knowledge of the continuum of care from ambulatory care to acute care to post-acute levels of care is preferred.

  • Staff will provide patient care, treatment, and services within the scope of their license, certification or registration and as required by law and regulation.

    • Evaluates patient condition.
    • Establishes treatment goals in collaboration with patient and family as appropriate, and in collaboration with the treatment team. Treatment goals should be interdisciplinary and patient-centered whenever possible. Essential Job Qualifications (Knowledge, Education, and Training Requirements):

  • Graduate of Commission on Accreditation in Physical Therapy Education (CAPTE)
