We are aware that rehab agencies have been experiencing denials for Medicare + Blue services over the past 2–3 months. We are actively working with BCBSM to resolve this issue and understand the additional financial burden this has placed on your clinics.

BCBSM recently updated its system and has identified a “glitch” that requires correction. We have communicated our concerns about the impact this is having on your clinics and on your ability to provide services to their members.

Rogers Introduces Resolution Opposing U.S. Department of Education’s Move to Remove Professional Designation From Health Care Degrees

LANSING, Mich., Dec. 12, 2025 — State Rep. Julie M. Rogers (D-Kalamazoo), a licensed physical therapist and longtime advocate for Michigan’s health care workforce, introduced House Resolution 226 this week urging the U.S. Department of Education (ED) to reject proposed changes to federal student loan regulations that would strip many advanced health care degrees of their “professional” designation.

The ED’s Reimagining and Improving Student Education’s (RISE) draft rule would make students in numerous clinical and behavioral health programs ineligible for the level of financial support historically available to them. Affected fields include advanced practice nursing, physical and occupational therapy, speech-language pathology, social work, physician assistant programs and other licensed professions.

“Michigan depends on competent, well-trained health care professionals, and we should not adopt changes that limit access to the degrees that make that possible,” Rogers said. “These programs provide rigorous professional training and have long been recognized as such. Preserving that designation ensures students can continue to access the financial aid they need to pursue these critical careers. Their education and licensure prepare them for complex, high-responsibility work, and our policies should continue to honor that reality.”

Rogers warned that the rule would make it harder and more expensive for students to enter high-demand careers, worsening provider shortages across Michigan’s hospitals, clinics, schools and long-term care facilities.

“We cannot afford policies that make it harder for students to pursue careers our communities urgently need,” Rogers continued. “Protecting access to these programs is essential to maintaining a strong, reliable health care workforce across Michigan.”

HR 226 urges the ED to amend the proposed rule and maintain the long-standing professional designation for these degrees to protect students’ access to essential financial aid.

“The RISE Committee’s proposed definition of a ‘professional degree’ does not reflect the reality of today’s healthcare,” said Aaron McCormick, President of the Michigan Nurses Association. “Excluding dozens of health professions from full loan eligibility would undermine the education pipeline and further strain access to essential services across Michigan. We support this resolution and call on the Department of Education to adopt an accurate definition that recognizes all licensed health professions requiring post-baccalaureate training.” 

It is with profound sadness that we share the unexpected death of Dr. Lucinda “Cindy” A. Pfalzer, a cherished physical therapist and leader within APTA Michigan, APTA Oncology, and the broader physical therapy and exercise science community. Dr. Pfalzer has left a legacy of scholarship, leadership, mentorship, and unwavering dedication to advancing the profession she loved.

A graduate of Ithaca College’s Physical Therapy Program in 1979, Dr. Pfalzer went on to earn a Master’s degree in Cardiac Rehabilitation and Adult Fitness from The Ohio State University in 1984, followed by a PhD in Exercise Physiology in 1989. Her academic training served as the foundation for a career marked by excellence, innovation, and service.

Dr. Pfalzer’s contributions to physical therapy are immeasurable. A Catherine Worthingham Fellow of the American Physical Therapy Association and Fellow of the American College of Sports Medicine, she exemplified excellence in clinical practice, education, and research. She served as a long-time delegate for Michigan to the APTA House of Delegates and was a past president of the Oncology Section of the APTA (now APTA Oncology), guiding the section’s growth and impact during pivotal years. She was instrumental in establishing oncology rehabilitation as a recognized and essential specialty within the profession and continued to serve as an onsite reviewer for oncology physical therapy residency programs. From 2013–2019, she also served as Editor of Rehab Oncology, further shaping scholarship within the field.

Dr. Pfalzer retired as a Professor Emerita at the University of Michigan–Flint, where she was named only the fourth Emeritus Professor in the department’s history—an honor reflecting her extraordinary influence on the program and its graduates. During her distinguished academic career, she authored dozens of peer-reviewed papers and book chapters and delivered more than 150 scientific and professional presentations worldwide. Her expertise spanned oncology rehabilitation, breast cancer, HIV/AIDS, multiple sclerosis, and upper quadrant overuse injuries.

Her career at the University of Michigan–Flint was marked by exceptional leadership. She served as associate director for research and post-professional education, director of the Transitional Doctor of Physical Therapy Program, and interim director of the Department of Physical Therapy. She played a central role in launching the campus’s first Doctor of Physical Therapy program and completed a prestigious oncology clinical research fellowship at the National Institutes of Health. Under her leadership of the post-professional certificate and residency programs, she facilitated the development of residencies in orthopedics, cardiopulmonary, neurology, and pediatrics across southeast Michigan—expanding high-quality professional development for countless clinicians. Beyond physical therapy, she contributed significantly at the school, college, and university levels, including service in senior leadership roles. Notably, she chaired the Self-Study Committee for North Central Association accreditation, supporting the maintenance of institutional accreditation at the University of Michigan–Flint.

Dr. Pfalzer’s research shaped clinical practice guidelines and informed health care reform efforts in oncology rehabilitation, improving care for countless individuals living with and beyond cancer. Yet perhaps even more impactful than her scholarly achievements were her gifts as a mentor. She invested generously in the growth of faculty, students, and clinicians, many of whom regard her as a defining influence in their professional lives. Through her vision, leadership, and steadfast commitment to excellence, she helped shape multiple generations of oncology rehabilitation specialists and profoundly strengthened the professional community we are privileged to share today. Her enduring legacy of mentorship will not be forgotten.

APTA Michigan, APTA Oncology, and the entire physical therapy profession mourn the loss of an extraordinary educator, leader, and visionary. Dr. Cindy Pfalzer’s legacy will endure in the clinicians she mentored, the patients whose lives are improved through her work, and the ongoing evolution of oncology rehabilitation—a field she helped build from the ground up.

We extend our deepest condolences to her family, friends, colleagues, and all who were fortunate to know her. May her memory continue to inspire us to pursue excellence, compassion, and innovation in service to our patients and our profession.

APTA President, Kyle Covington released a statement yesterday (11/24/25) and call to action on the recent recommendation from the Reimagining and Improving Student Education (RISE) Negotiated Rulemaking Committee to designate the doctor of physical therapy degree — and dozens of other health care professional degrees — as a graduate degree over a professional degree. 

APTA strongly disagrees with the recommendations of the RISE Negotiated Rulemaking Committee that suggest the doctor of physical therapy degree is a graduate degree, not a professional degree. Read the full APTA statement.

Follow these guidelines when billing for remote patient monitoring

Remote patient monitoring allows patients to collect their own health data using internet-connected medical devices that automatically transmit the data to their health care providers. Data could be on blood pressure, weight or glucose levels, for example. The providers then use this data to treat or manage patients’ conditions.

RPM consists of three main components, each building off the step before it:

1. Patient education and device setup
2. Device supply
3. Treatment management

To correctly submit an RPM claim and avoid payment delays, you need to follow specific steps and ensure compliance with billing and documentation requirements.

The change in policy is the result of sustained and strategic effort from APTA staff and the engagement of members spanning almost a decade and marks a significant victory for patients and for the profession.

This meeting is scheduled for Tuesday, July 15, 2025, from 12:00 p.m. to 1:30 p.m. via telephone and Microsoft Teams.

Cindy Kincaid Outstanding Clinical Instructor Award

The Cindy Kincaid Outstanding CI Award provides recognition and encouragement to clinical instructors who have demonstrated exceptional skills in teaching, communication, interpersonal relationships, professional competence, mentoring, modeling, and professionalism.

The nominated PT or PTA must have supervised a full time student, be licensed in Michigan and be an APTA/APTA Michigan member. The nomination must be initiated by a student in their final year of a PT or PTA program; forms are available by contacting the ACCE/DCE of the student’s academic program or on the APTA Michigan website under Resources/Awards.

Norene Clouten Outstanding Site Coordinator of Clinical Education Award

Purpose: To acknowledge individuals who have made significant contributions to physical therapy clinical education through excellence in coordination of clinical teaching.

Nominees must be a physical therapist (PT) or physical therapist assistant (PTA) who has been actively engaged in the coordination of clinical education for at least 2 years.    Nominees must be a Site Coordinator of Clinical Education (SCCE) that affiliates with a Michigan PT or PTA education program.    Nominees must supervise clinical instructors who work directly with students from accredited PT or PTA programs.  PT/PTA recipients of the award must be members of the APTA Michigan/APTA.

Any individual, clinically, or academically based, with knowledge of the SCCE’s abilities to coordinate clinical education activities and supervise and teach clinical instructors may submit nominations for the award. The nomination form included below in our file library. 

The deadline for the receipt of the nomination form and the supporting documents is June 1. 

APTA Michigan has been very active behind the scenes regarding BCBS’s MPPR implementation, some of which we are not able to share.  This letter is an example of what we sent along this week as we continue to hear silence from BCBS on this important issue.

From the letter: “APTA Michigan has communicated and met with BCBSM multiple times and provided research-backed documentation outlining the devastating effects of this policy. Responses have been minimal. We are aware of—and strongly support—the hundreds of letters sent to Liz Bowman and Dr. Faris Ahmad at BCBSM requesting a reversal of this harmful policy.”

In addition to this letter, our Payment Specialist, Barb Herzog contributed to a recent article in the Detroit Free Press.  "We save them money," said Barbara Herzog, a payment specialist with the American Physical Therapy Association, an industry group. "But this (reimbursement cut) is making our profession almost unsustainable."

Legislation to Improve Ease of Access to Physical Therapy in Michigan Introduced

Michigan State Senator Sylvia Santana (D-Warrenville) has introduced Senate Bill 144 which aims to improve ease of access to physical therapy by removing restrictions and barriers to a patient seeking physical therapy services.

Senator Santana remarked: “Removing barriers to care is important to ensuring Michiganders have the ability to receive healthcare they need.”

All 50 states allow an individual to seek treatment from a physical therapist without a physician referral or prescription. However, some states, including Michigan, have excessively restrictive provisions that limit access. Currently, there are 21 states with unrestricted direct access. Nebraska, being the first, has allowed this since 1957. 
 
Patient data from these states demonstrated that when citizens access physical therapy services directly, without needing to schedule an appointment with their physician first, and without restrictions, the overall cost of care and burden to the health care system was lower. 

"I'm excited and hopeful to see the continued support of legislature to move SB 144 forward as it was moving this way through the last legislative session.” Stated APTA Michigan President Brian Gilbert.  “Thank you so Senator Santana for getting this going once again. This bill will make such a difference for those seeking care, but also for our providers to reduce the stress of the existing restrictions on practice and attract and retain amazing talent to treat our communities."

Senate Bill 144 was approved by Senate Health Policy Committee yesterday and referred the Senate Floor for consideration.  APTA Michigan would like to offer a huge thank you to the Senators on the committee for voting to pass this bill out of committee.  APTA Michigan is grateful for the leadership of Senator Santana and looks forward to working collaboratively to get this legislation signed into law.

Legislation Enabling Michigan to Join the Physical Therapy Interstate Licensure Compact Reintroduced

Michigan State Representatives Matthew Bierlein (R-Vassar) and John Fitzgerald (D-Wyoming) have introduced House Bills 4101 and HB 4102, legislation that would add Michigan to the Physical Therapy Interstate Licensure Compact. This important legislation would enable Michigan physical therapists to obtain multistate licensure to practice physical therapy in other states that also participate in the compact without having to obtain a separate license in each individual state.  

The legislation helps facilitate the interstate practice of physical therapy with the goal of improving access to healthcare. The bills also allow the state to preserve its regulatory authority and protect the health and safety of residents through current systems of state licensure. The interstate practice of physical therapy makes it easier for physical therapy providers in Michigan to ensure the physical therapy needs of rural and underserved areas are met and to maintain continuity of care for their patients living and travelling outside of Michigan through telehealth. The compact could also serve as an important tool in attracting and recruiting physical therapists specializing in critical shortage areas of care such as women’s health, chronic pain, long COVID, and oncology rehabilitation.

If passed, Michigan would join 38 other states, including Great Lakes neighbors, Wisconsin, Indiana, and Ohio who are already part of the compact.

“These bills help reduce the burden of doing business in Michigan, making it easier for new PT's to choose practicing here,” said State Rep. Matthew Bierlein (R-Vassar), sponsor of House Bill 4101.  “We want to be a destination for people and having access to quality medical care is so important when making decisions on where to live. This is just one more tool in the box to help us improve and grow this great state.”

"Access to physical therapy professionals is important, especially in underserved and rural areas where healthcare services can be sparse," said State Rep. John Fitzgerald (D-Wyoming), sponsor of House Bill 4102. "By joining the Physical Therapy Licensure Compact, we can enhance the ability of physical therapists to serve across state lines, broadening the availability of essential health services and ensuring that citizens receive timely and effective care. By passing these bills, we are not just reforming licensure; we are reaffirming our commitment to the health and well-being of every Michigander."

House Bills 4101 and 4102 have been referred to the House Health Policy Committee for consideration. APTA Michigan is grateful for the leadership of both Representatives and looks forward to working collaboratively to get this legislation done.

Representative Julie Rogers deserves our heartfelt appreciation and gratitude for her hard work and passion in advocating for SB 668. She was a tremendous advocate. Most individuals don't understand the work that goes on behind the scenes, and I can speak for the work that Representative Julie Rogers has done, most of which will go unnoticed and unappreciated. If you see her, know her, or feel compelled, make sure to thank her for being the 1st PT in Michigan to serve as a legislator and being the ultimate advocate for physical therapy.

As a final note to this information. I want to thank the those that helped us in this fight this year. I am honored to be a part of this profession and to fight for what is right for the people of this State. I believe in what we did this year and it will be our top priority in the coming legislative session. I also want us to start working towards legislative action around payment and reimbursement. The October Record announced the implementation of proposed Multiple Therapy Services/Therapy Pay Percent reduction (MPPR) policy effective 3/1/2025.  BCBSM will apply “a reduction in reimbursement for the practice expense portions of therapy procedures when those services are the secondary or subsequent procedures provided on a single date of service”.  APTA MI, working with APTA National along with hundreds of physical therapists and provider groups, responded with a letter writing campaign to BCBSM. These collaborative letters cited this reduction as flawed and inappropriate as well as devastating to the survival of both independent providers to large corporations.  We asked that BCBSM recognize the value of this profession, ensure continued access to care for their members, and provide appropriate payment for these services. Additionally, it was unclear who this policy would affect—just “Facilities” or “Professionals” indicating all provider types.  Although APTA Michigan asked for clarification from BCBSM as to timing of implementation and provider type, we did not receive a response. We did, however, receive a copy of several emails that some members received, for which we interpret “professional providers and facilities” to mean all providers:

Contract Administration Process BLUE CROSS BLUE SHIELD OF MICHIGAN RESPONSE: Multiple Therapies Reimbursement Policy

Based on Blue Cross Blue Shield of Michigan’s review of the Multiple Therapies Reimbursement Policy responses, the implementation of this policy will be moving forward through the approval process. A tentative implementation date is scheduled for March 1, 2025, for professional providers and facilities.

As a reminder, the reduction only comes on the practice expense portion of therapy services which are the secondary or subsequent procedures provided on a single date of service by the same group physician and/or other qualified health care professional. This policy does not apply to therapy services rendered with a behavioral health diagnosis.

The CAP for Multiple Therapies Reimbursement Policy will be completed with presentation at the December 11, 2024, Staff Liaison meeting and at theJanuary 30, 2025, PHA Advisory Committee. An article will be published in the March 2025 edition ofThe Recordreflecting the outcome of this process.

APTA MI is working hard to engage with Liz Bowman to continue to express our grave concern and opposition to this policy. Even CMS has defined the MPPR as duplicative and inappropriate.

We welcome your input, information about any communications with BCBSM, and ideas for action that may assist in a resolution and stop implementation of this devastating policy.  BCBSM needs to HEAR us!

BCBSM is projecting implementation of its MPPR policy on 3/1/25.  Input to BCBSM is requested by 10/31/24.  The Record has this listed under "Facilities" and it is unclear if it will include "Professionals" as it has in the past.  APTA payment committee has reached out to BCBSM for clarification but has not received a response.  We encourage your action on this policy which is predicted to reduce payment approximately 17-20%.
